SCOUT UNIFORM BSA Forms Troop DocumentsScouts and Parents: All scouts are required to submit a Medication Form in a ziploc bag with the scouts medicine to the Scoutmaster (SM) before any campout or day activities where medication is required. If the SM is not available, please provide the form and medicine to the Assistant Scoutmaster (ASM) in charge of the activity. Scoutmasters are not allowed to administer medication without a signed medication form. Only the medicine detailed on the form can be administered by a SM or ASM. Also, any scout that has been sick or under medical care during the week before camp must notify the SM or ASM in charge about their condition and what activities they are limited to. Please help us with this process so we can assure your safety at camp or activities.
